Hello Siuslaw families,
The Siuslaw School District has enrolled in the OHA/UO COVID-19 Screening Testing Program this year. This is an optional, weekly saliva test, via a take-home test kit. Please review this info/registration-flyer for more information.
If you’d like to participate, please sign up by completing these two steps:
STEP 1: Complete the UO K-12 Student Information Survey: https://tinyurl.com/k12covidtesting The registration survey can be viewed in English or Spanish. The Oregon Health Authority has a rolling enrollment; students can sign up anytime. New enrollees are processed each Sunday. Testing can be expected to begin about 1-2 weeks after student sign-up begins.
STEP 2: Complete and return the OHA Consent Form. This form is required for all participating students and must be completed and returned to the school at the start of the program – plan to bring this form on your first test pick up day. If you don’t have a printer, then your school can provide a printed copy at test pick up. Schools are required by OHA to retain these consent forms.

La encuesta de registro se puede ver en inglés o español. La Autoridad de Salud de Oregon tiene una inscripción continua; los estudiantes pueden inscribirse en cualquier momento. Los nuevos inscritos se procesan cada domingo. Se puede esperar que las pruebas comiencen aproximadamente 1-2 semanas después de que comience la inscripción del estudiante.
PASO 2: Complete y envíe el formulario de consentimiento de OHA.
Este formulario es obligatorio para todos los estudiantes participantes y debe completarse y devolverse a la escuela al comienzo del programa; planifique traer este formulario el primer día de recogida del examen. Si no tiene una impresora, su escuela puede proporcionar una copia impresa al recoger el examen. La OHA requiere que las escuelas retengan estos formularios de consentimiento.
