The criminal background check may take up to 2 weeks; please plan as necessary!
Proof of vaccination – Volunteers are included within the state’s vaccine mandate – for regular or reoccurring volunteers this is the easiest way to be prepared to volunteer.
Please be prepared to show a Covid-19 vaccination card that shows you are fully immunized; which is two weeks past their final dose.
A photocopy of the vaccine card will be kept on electronic file at the district office.
Volunteers that are unvaccinated must show a Negative Laboratory Test Result that is dated within 3 days prior to the volunteer event.
A negative Covid-19 test is only good for events within a 3 day window of that test.
Subsequent field trips or activities will require additional negative test results.
Local pharmacies have good quality, reliable, over the counter tests available

Laboratory tests typically take 2-3 days to process and get results back to you.

VOLUNTEER APPLICATION PACKET - Instructions for submission to be an approved volunteer
The Siuslaw School District is in need of volunteers who enjoy working with children and are willing to help out a few times each month during the school year. No teaching experience is necessary. Classroom teachers utilize volunteers for reading groups, material preparation, chaperoning field trips and more!
Please complete the following steps. The entire process should take approximately 15 minutes.
Step 1: Read Volunteer Handbook
Step 2: Fill out the On-line Application and Background Check -Background checks are good for two years.
Step 3: You will receive an email once you are approved. You are welcome to contact the volunteer coordinator at 541-997-2651, if you have any questions or concerns about the process.
If you would like to learn more about volunteering at Siuslaw, you are welcome to attend an orientation at the school district office. This orientation will include a question and answer time along with a tour of the school. Please contact Vonnie McClellan at 541-997-2651 for more information.
PRIVATE DRIVER FORM
This form is for drivers of private vehicles used for Siuslaw School activities, including 97J employees. The Private Drive Form and additional information is required for any individual (volunteer or staff member) driving students other than their own to a district sponsored event.
Parents/guardians may transport their own child in a private vehicle with prior written notification to the coach and/or building administrator when district transportation is not available. The Private Driver Form is not required for parents/guardians driving their own child.
Follow these steps:
Driver must be an approved volunteer or staff member; application process is outlined above.
A new Private Driver Form is required each school year.
Provide a copy of your driver's license.
Provide a CERTIFICATE of Liability Insurance Form from your insurance agent. Your insurance agent can email or fax it to the district office for you. It can be sent by fax to 541-997-6748 or email to vmcclellan@siuslaw.k12.or.us

Important -- Submit Your Hours
It is very important that you submit the hours that you volunteer. Please choose one of the following methods for submitting this information to the district:
1) Sign-in at each school office. Volunteer Hours may also be tracked on computers in each of the school offices.
2) Home Access Click here to enter your volunteer hours from home.
3) Volunteer Hours Report To report hours manually, fill out this form and return to the head coach or athletic director. The report will be filed with the district office.

S.M.A.R.T. - Start Making A Reader Today
SMART volunteers meet on Wednesdays from 1:45 to 2:45 at the elementary school. Volunteers are needed to read with two children for half an hour each from a variety of children's fiction and non-fiction books. They will provide a positive, one-on-one adult interaction by reading to, listening to, or reading with students. Education and Training Beyond High School
ASPIRE is a mentoring program that matches trained and supportive adult volunteer mentors with middle and high school students to develop a plan to help them meet their education goals beyond high school. Click here to learn more about the ASPIRE Mentoring Program.
Once trained, volunteers will continue on with a handful of advisees until they graduate, usually meeting bi-weekly with juniors and quarterly with sophomores.
