Community Facility Use Requests
Facility use requests must be submitted through a Google form using the links below. One link is for one-time use events and the multi-days link will be for ongoing use through the school year. Requests will be considered on a first come, first served basis with school activities taking priority. Use of facilities during school hours (typically 8:00am – 4:30pm) is not allowed. Please read Policy KG – Community Use of School Facilities and the accompanying administrative rule for details and procedures. Please note that charges may apply and *insurance is required for all applicants.

* Insurance Requirements
All applicants will be required to provide proof of general liability insurance coverage in the form of a Certificate of Insurance. In addition to the general liability insurance coverage, the insurance coverage must also include the automobile liability endorsement in the event of any accident involving a vehicle of the applicant group while on district grounds; automobile liability will be designated at $1,000,000 combined single limit per accident. This certificate must show that the insurance coverage will be in effect during the event date(s) and show minimum general liability coverage in the amounts of $2,000,000 per occurrence and $2,000,000 in aggregate liability coverage, with automobile liability stated at $1,000,000. The school district must be named as additionally insured on said policy and a copy of the endorsement is to be included with the insurance certificate. Coverage cannot be canceled or reduced without 30 days written notice to the district. The superintendent may require additional insurance depending on the nature of the facility use.
