Siuslaw School District Special Education Department, will be destroying Special Education records of:
Students whose special education services with Siuslaw School District ended prior to 6/15/2018 and who were born before 8/31/2002.
(Records are required to be retained until the student reaches age 21 or 5 years after the conclusion of Special Education services, whichever is longer as per OAR 166-400-0060)
Students/Parents preferring to keep their records may arrange to pick them up by calling the Special Programs Department office at 541-997-2514 ext 5444. An individual 18 years of age or older, unless adjudicated, must request his/her own records. Proper identification is required. Above records will be destroyed after June 15, 2024.
