The mobile community health services team from Oregon Integrated Health will be at Siuslaw Middle School (2525 Oak Street) to complete a free sports physical exam for any student that may need one for this upcoming school year. The Oregon Integrated Health Mobile Services Clinic facility will be in the parking lot of SMS.
Free Sports Physicals will be offered one more time, tomorrow August 14th - Thursday 8/14 from 4pm to 8pm
