General Facility Use Liability Release Form - In addition to the Facility Use Request Form used for scheduling, there is an additional requirement for all groups requesting use of school facilities to complete the General Facility Use Liability Release Form.
Completed Facility Use Forms should be returned to the building being requested. The building administrator will confirm availability, give building approval and send the form to the district office for final approval and fee assignments.
